It's just a form, I dont see what there is to review unless you're talking about forum categories and it's skin.

I'm not especially a fan on the skin, I see a little bit of the background underneath the ad because I'm using Mozilla. And the categories look fine to me. If you're just starting out the form, I wouldn't recommend adding any categories until the stats pick up a little.
