Looking for real phone support

I'm trying to find a reseller with the following requirements. If you know of one, please post a reply:

* Actual phone # on support page (at least an emergency one)

* $25 mo. or less, billed monthly.

* USA based operation

* Company in business at least 1 yr

* At least 600 Mb disk space, 15 Gb xfr (5 -10 domain limit OK)

* CGI, SQL, and the usual stuff. Shared SSL would be a plus.

* Password protection for folders

* At least 99.7% uptime
