Needs to be ASP or ASP.NET Can Use a MSSQL or ACCESS DB if necessary, but i dont think it needs it, since the emails and info does not need to be saved.

I can pay a couple bucks for this if necessary. It shouldnt take long -the base is there -you just need to make it combine data to make a full email address.
