I know 2advanced.com is good but they start $20'000 per site, so we'll pass them.

I worked with Ravio.com - they are pretty good.
Cool banner makers - GetaBanners.com, despite their crappy domain, they do really cool banners. Check this one out they made for me this one. Only $20!
