Speaking from a personal point of view here because everyone has their own market but I would not recommend a maintenance fee "included" in the hosting fee...that really shoudl be seperate.
Im afraid what you would find is clients that want constant changes, work done and you certainly wouldnt see too much profit in doing that after awhile.
